Magnesium & Women’s Health After 40: What Your Labs Reveal

Magnesium is involved in over 300 enzymatic reactions in the body — from energy production and muscle function to sleep regulation and bone metabolism. Yet magnesium deficiency is one of the most common and most underdiagnosed nutritional deficiencies in adults, particularly women over 40.1
Here's what makes it especially tricky: the standard blood test for magnesium (serum magnesium) is a poor indicator of true magnesium status. And most clinicians don't order it routinely anyway.
Why Standard Labs Miss Magnesium Deficiency
Only about 1% of the body's total magnesium is found in the blood. The rest is stored in bones, muscles, and soft tissue. Your body tightly regulates serum magnesium by pulling from these stores — so your blood level can appear normal even when your cells are depleted.2
A more accurate test is the RBC magnesium (red blood cell magnesium), which reflects intracellular magnesium status. If you suspect deficiency, ask your doctor specifically for this test rather than standard serum magnesium.
Why Magnesium Matters More After 40
Several factors converge after 40 to increase magnesium needs and reduce magnesium status:
- Declining estrogen — Estrogen helps retain magnesium. As estrogen drops during perimenopause, magnesium excretion increases.3
- Bone remodeling — Magnesium is essential for bone density and works synergistically with calcium and vitamin D. Low magnesium impairs vitamin D activation.4
- Sleep disruption — Magnesium regulates GABA, the calming neurotransmitter. Deficiency is associated with insomnia and poor sleep quality — a common complaint in perimenopause.
- Cardiovascular health — Magnesium supports healthy blood pressure and heart rhythm. Low levels are associated with increased cardiovascular risk.5
- Blood sugar regulation — Magnesium plays a role in insulin sensitivity. Deficiency is associated with increased risk of type 2 diabetes.6
Signs of Magnesium Deficiency
Because deficiency is rarely caught on standard labs, symptoms are often the first clue:
- Muscle cramps or twitches (especially at night)
- Poor sleep or difficulty staying asleep
- Fatigue and low energy
- Headaches or migraines
- Anxiety or irritability
- Constipation
- Heart palpitations
Which Form of Magnesium Is Best?
Not all magnesium supplements are equal. The form matters significantly for absorption and therapeutic effect:
- Magnesium glycinate — Highly bioavailable, gentle on the gut, best for sleep, anxiety, and general deficiency correction. My top recommendation for most women.
- Magnesium malate — Well-absorbed, supports energy production (malate is involved in the Krebs cycle). Good for fatigue and muscle pain.
- Magnesium citrate — Good bioavailability, mild laxative effect. Useful for constipation but not ideal for high-dose supplementation.
- Magnesium oxide — Poorly absorbed (~4% bioavailability). Commonly found in cheap supplements. Avoid for therapeutic use.7
- Magnesium threonate — Crosses the blood-brain barrier. Emerging research suggests benefits for cognitive function and memory.8
How Much Do You Need?
The RDA for magnesium for women over 31 is 320 mg/day.9 Most adults get significantly less than this from diet alone, particularly those eating processed foods. Supplemental doses of 200–400 mg/day of elemental magnesium are commonly used, but work with your healthcare provider to determine the right amount for you.
Food Sources of Magnesium
Before supplementing, optimize dietary intake: dark leafy greens (spinach, Swiss chard), pumpkin seeds, almonds, black beans, avocado, and dark chocolate are among the richest sources.
